Following surgery, the most successful recoveries happen during a short stay in a comfortable setting, with onsite therapy and rehab professionals. It’s the best possible scenario because it eliminates scheduling and transportation challenges so elders can focus on healing in a peaceful, relaxed environment.
With only 12 elders in each cottage, and top-notch infection control practices, the likelihood for infection transmission at Green House® Cottages of Wentworth Place is substantially lower than at traditional facilities. This is a huge plus when recovering from surgery because when you sign up for a short stay, the last thing you want is an infection to impede progress and lengthen healing time.
All elders, no matter the length of their stay, enjoy all the amenities, services, and compassionate care:
- Clean, comfortable, climate-controlled private rooms and bathrooms
- Daily Housekeeping
- Laundry Services
- 24/7 Made-to-Order Meals
- 24/7 Onsite Personal Care
- 24/7 Onsite Nursing Care
- Full Access to Therapy & Rehab Services
How Long is Short?
A short stay can last a week, eight weeks, or somewhere in between. It varies depending on the person, the complexity of their medical condition, treatment plan, and ability to heal. A team of medical professionals – doctor, nurse practitioner, director of nursing, MDS coordinator, social services representative, and rehab therapists – meet regularly to strategize how to help the elder recover, be discharged, and return home.
From Hospital to Recovery
When elders come to Green House® Cottages of Wentworth Place following a hospitalization for a surgery, procedure, or diagnosis, they find the best care possible at our skilled nursing rehab center. We have all the experience, expertise, and infrastructure in place to help elders recover after orthopedic procedures, stroke, heart attack, amputation, injuries, acute illness, and more.
Short-Term Care Services
We provide a variety of short-term care services, including:
Rehabilitation
Short-term care at a Wentworth Place includes access to rehabilitation services such as physical therapy, occupational therapy, and speech therapy. These services are tailored to help individuals regain strength, mobility, and independence after surgery, injury, or illness.
Medical Care
Our skilled nursing care is provided by licensed healthcare professionals, including registered nurses, licensed practical nurses, and certified nursing assistants. Medical services may include wound care, medication management, monitoring vital signs, and administering treatments as prescribed by healthcare providers.
Post-Surgical Care
Individuals recovering from surgery, such as joint replacement, cardiac surgery, or major procedures, receive specialized post-surgical care at Wentworth Place. This may include wound care, pain management, rehabilitation therapy, and assistance with activities of daily living.
Pain Management
Pain management is a priority in our short-term care, with healthcare providers working closely with residents to ensure effective pain control and comfort during the recovery process. This may involve medication management, non-pharmacological interventions, and supportive care measures.
Transitional Care
Short-term care serves as a transitional bridge between hospitalization and returning home or to a lower level of care. Our nursing home staff coordinate with healthcare providers, caregivers, and family members to develop a personalized discharge plan and ensure a smooth transition back to the community.
Assistance with Activities of Daily Living (ADLs)
Residents receive assistance with ADLs, such as bathing, dressing, grooming, toileting, and eating, as needed during their recovery period. Caregivers provide support and encouragement to help residents regain independence in these tasks.
Nutritional Support
Nutritious meals and snacks are provided to support residents' recovery and overall well-being. Dietary preferences, restrictions, and therapeutic diets are accommodated to ensure residents receive adequate nutrition and hydration during their stay.
Family Involvement
Family members are encouraged to participate in the care planning process, visit regularly, and provide support and encouragement to residents during their recovery. Open communication between staff and families ensures residents receive the necessary support and resources for a successful recovery.
Discharge Planning
When it’s time for an elder to return home, our knowledgeable staff can help with all aspects of the transition, including an evaluation of current living conditions and recommendations for modifications and setting up equipment to support safe and comfortable independent living.
Our staff work closely with residents, caregivers, and healthcare providers to develop a comprehensive discharge plan that addresses ongoing care needs, medication management, therapy services, equipment needs, and community resources. The goal is to ensure a safe and successful transition back home or to a lower level of care.
