How Long Does Short-Term Care at Wentworth?
The goal of short-term care is to get back home as soon as possible while keeping safety and wellbeing in mind. Everyone is different, and the length of stay depends on treatment and medical complexity.
Wentworth short-term rehab patients have individualized care plans that include meetings with a medical doctor, nurse practitioner, director of nursing, MDS coordinator, social services, and rehab therapists. Individual plans can range from one week to eight weeks or more depending on progress.
Home Care Support
The Wentworth staff can provide help with the transition back home by evaluating home living conditions and recommending modifications to make independent living safer and more comfortable. We can set up needed equipment, access modifications, and assist with a seamless transition.
